The story about Josiah
On November 3, 2017, Monica Hinojosa, came in to Physicians Premier Emergency Room Calallen with complaints of abdominal pain.
She was 25 weeks pregnant and suspected that her water had broken.
Shortly after arrival, she spontaneously delivered an extremely delicate preterm baby boy who was not breathing. The emergency room team successfully resuscitated and called 911 for immediate transport and transfer to Bay Area Hospital where a group of physicians and nurses awaited baby. He stayed a few months in the Neonatal Intensive Care Unit (NICU) at Bay Area. The Hinojosa family was fortunate to get to their closest ER in time for what is referred to medically as a precipitous labor delivery. Baby presented breach and without the immediate presence of emergency medical expertise, would not likely have survived.
The Hinojosa family extend their deepest thanks to the ER staff on duty that day, including the first responders at the Annaville Fire Department.

Authorization for use/disclosure of information and consent for use audio / video/ photograph images under the HIPAA – Health Information Portability and Accountability Act of 1996 Physicians Premier Emergency Room (PPER) is always pleased when patients are willing to communicate and share the stories, experiences, and information about their treatment received at our emergency room facilities. Sharing your story can help others who are interested in knowing more about the emergency care services provided by Physicians Premier ER care providers and can help PPER promote its mission of service. Physicians Premier ER respects the privacy of our patients, visitors, and staff. Ensuring that medical information is kept confidential is among our highest priorities. By your submission, PPER accepts your permission to use your shared medical information via audio/video/photographic material provided by you in Physicians Premier ERs internal and external communications, including medical and general interest publications and medical and patient education information, and distribute such materials online and in news media (such as TV, radio, newspapers, and magazines). To ensure that PPER is acting in accordance with your wishes, and using your personal information with your submission confirms that you: Do give permission for Physicians Premier ER to use your name and share details of your treatment and experience as a patient in communications produced by or on behalf of PPER, and consent to take and make use of your audio/ video/ photographic images in media communications produced by or on behalf of Physicians Premier ER. This permission extends both to electronic versions on the Physicians Premier websites and other internet/electronic applications as well as to printed, filmed, and recorded versions. Do give the right to request that audio/video recording, filming, or photographing cease at any time. I am aware that my protected health information will exist forever in either a recorded, printed, and/or electronic version or other version as may develop over time and that once it is published or disclosed in any form it will continue to be used. Do understand that information about me disclosed per this authorization may be subject to re-disclosure by the recipient and will no longer be protected by the federal regulations protecting privacy of an individual’s health information under the Health Information Portability and Accountability Act of 1996 (“HIPAA”) and other applicable federal and state law.
